Physician Referrals

Thank you for choosing Infinity Therapy for your patient’s speech, language, cognition, swallowing, or voice care. To refer a patient for evaluation or treatment, complete the Infinity Therapy Referral form below and fax it to 501-313-0380. Please send it along with the patient’s insurance card and recent medical history. Infinity Therapy will then contact the patient directly to arrange an appointment.

After the assessment, the referring provider will receive a detailed Plan of Care and ongoing progress updates to ensure coordinated, person and family-centered care.

New Patients

If your physician has recommended or referred you for a speech-language pathology evaluation, please call us at 501-909-4123 or email info@infinitytherapy.org to schedule your appointment.

Before your first appointment, please ensure we receive:

• A copy of your insurance card

• Your medical history or a recent physician note

• Your physician referral form (typically faxed directly from your provider)

If you do not yet have a referral, we can help. Contact us to discuss your symptoms, and our team can work with your physician to determine whether an evaluation is appropriate. Once your referral and documentation are received, we’ll schedule your initial assessment.

Infinity Therapy accepts Medicare, Medicaid, Blue Cross Blue Shield, United Healthcare, Humana, and Aetna insurance plans. Please contact us to verify your benefits and coverage before your appointment.
